What to bring to the first discussion

You do not need to arrive with a complete medical file. A short account of the treatment and the concerns you want investigated is a useful starting point.

The treatment

Note where you were treated, the approximate dates, the procedure or advice involved and what happened afterwards. Include any later treatment or revision procedure.

The documents

Bring relevant letters, discharge summaries, appointment records and correspondence already in your possession. Say whether you have requested your full records.

The effect on you

Explain the symptoms, further treatment, time away from work and day-to-day difficulties. Bring photographs or expense records where they are relevant.

Discuss the investigation and costs before instructing

Ask what work is proposed, which records are needed and whether an independent expert report should be obtained. It is also important to discuss the cost of that work, including payments to other providers, before authorising it.
